Notice to Patients of Longwood Family Health of Patient Record Incident

Protecting the privacy and security of your personal information is a very important priority of
Florida Hospital Medical Group (FHMG). While transferring boxes from our storage vendor,
Access, to another one of our storage vendors, Iron Mountain, we discovered on or about August
17, 2016, boxes containing your patient information were inadvertently misplaced. We believe
these boxes are most likely located in either Access or Iron Mountain’s secure facility. However,
because we are unable to locate them at this time we are notifying you of this event out of an
abundance of caution.
 
The paper medical records included in the containers are dated from 2003-2013. All patient
records have been stored electronically since 2014. The medical records may contain names,
visit date(s), date of birth, insurance information, laboratory results, referrals, x-ray images,
Social Security numbers, billing information, patient history, consent forms, and copies of
driver’s licenses.
FHMG performed an internal investigation into this matter. We are requiring the vendors to
continue to search for the containers storing our patients’ records. As part of our corrective
action and to prevent further incidents, FHMG is reviewing the processes our vendors use to
safeguard our patients’ records and will hold them accountable to these new processes.
 
While we have no evidence that patient information has been misused, we are taking all steps
necessary to help guard against the potential damage this may cause. We have secured the
services of Kroll to provide identity theft protection at no cost to impacted patients for one
year. Kroll is a global leader in risk mitigation and response, and their team has extensive
experience helping people who have sustained an unintentional exposure of confidential data.
Identity theft protection services include credit monitoring, identity consultation, and identity
restoration.
